Imaginary friends turn back into dolls when their sim friend is asleep, and will usually sleep near their sim's bed.

Your sim will have to keep up a good relationship with his or her imaginary friend and interact with it a often so that it grows up. ImaginaryFriend-MarchAndDoll.jpg
When your sim is a toddler, their future imaginary friend will be only a doll. This is the point at which you can decide whether you want a fully-fledged imaginary friend or not. Toddlers can play with or sing to their dolls, developing a relationship between the toddler and the doll. If the relationship between the two is high enough, the doll will "grow up" when the toddler ages up to child stage.

With Children

Surprise!
Soon after your sim toddler grows up into a child, a popup that will suggest to take the doll out of your sim's inventory and put it on the floor, provided their relationship was high enough. When the doll is placed on the ground, it will "grow up" into a larger form. This is the point at which the doll stops being a doll and becomes an imaginary friend that your sim can interact with.

The doll can now walk around and interact with objects and sims. It has free will as if it is a visitor. It is visible only when the owner of the doll is selected – with any other sim selected, you won't be able to see it. Imaginary friends turn back into dolls when their sim friend is asleep, and will usually sleep near their sim's bed.

Children can perform most of the same interactions with their imaginary friends as they can with other sims, including chatting, playing, joking, and so on. They can also play with and sing to their imaginary friends while they're in doll form, just like when they were toddlers – but you have to queue up the action while your sim is asleep, while the imaginary friend is in doll mode. Kids can also play with other children's imaginary friend dolls, even though they can't see them when they're "real".

Kids can also order their imaginary friends to go get them a snack, or to clean up after them – with varying degrees of success.

With Teens

When your sim child grows up to be a teen, the imaginary friend will return to being just a doll unless it has been made real. However, the doll can still be used by other toddlers and children.
